In order for your athlete to participate on the team you will need to obtain medical approval from a doctor or nurse practitioner. Approvals from chiropractors are no longer accepted.

You must register your athlete on RegisterMyAthlete before they can participate in any activities with the team. The Cabrillo Middle School Athletic Clearance Instructions explain all of the necessary steps in detail.
